CARER CONNECT

Carer Connect is a program of flexible one on one support for carers of people who have a psychiatric disability, with a focus on ageing carers.

What do we offer?

• Home visits
• Emotional support
• An assessment of your needs to determine how we can best help
• Working with you to develop a personalised care and support plan

Who is eligible to use Carer Connect?

Persons living in the greater Brisbane area who are voluntarily caring for someone with a psychiatric disability

How much does it cost?

The service is free to those who are eligible.  However referrals may be made to other agencies which could attract a fee.

Carer Connect is funded by Disability Services Queensland.

'What will happen when I can no longer care for my adult child with a disability?'

Disability Services Qld provided funding to develop a 'blueprint' for a model of service that will assist families to plan for the future care and support needs of their family member with a disability.
